With Concord's Operational Performance Information System (OPIS), you can mine the wealth of knowledge gained from operating experience - both failures and successes - and make it instantly available to the right people in your organization.
You can collect, analyze, and immediately disseminate lessons learned from incident reports, root cause analysis, maintenance histories, and the numerous other sources of operational data available from you plant and throughout the industry. You can become a "learning organization", continually analyzing operational experience, identifying opportunities for enhancement, and implementing improvement recommendations.

The table below lists the key OPIS tools for promoting feedback and learning. We will be adding additional tools soon, and keep in mind that you may use any of these tools as-is, or we can customize the format and behavior to better fit your organization. Also, we can add additional reports/tools that you may already be using.

Performance Improvement Recommendations

Performance Improvement Recommendations (PIRs) are suggestions submitted by employees with the purpose of improving safety and operations.

Near Miss Reports
|
Near Misses are reports of circumstances in our operations that could have led to an accident but did not. Reporting near misses will help others in the company become aware of such situations so that they can make improvements to help reduce the likelihood of similar occurrences.

Lessons Learned Reports

Lessons Learned Summaries are designed for the specific purpose of disseminating information about the safety and operations lessons we have learned in the course of our operations.

Incident Reports
|
Incident reports document the occurrence of process safety and operations incidents. Subsequent investigation of such incidents and forthcoming recommendations are intended to help promote safer operations and prevent future incidents.

Corrective Actions
|
Several efforts documented within OPIS (e.g., Lessons Learned, Near Miss Reports) generate recommendations for improvement. This area of the OPIS site provides direct access to these various recommendations and allows browsing, searching, and updating status.
